New District User Setup form for setting up new district user accounts.

First Name:
The first name of the contact person who will be entering the purchase amounts.
Last Name:
The last name of the contact person who will be entering the purchase amounts.
E-mail:
The e-mail address of the contact person who will be contacted concerning the entered purchase amounts. The user must supply a valid e-mail address for DOE contact purposes.
Phone:
The full phone number, including area code, of the contact person who will be contacted concerning the entered purchase amounts.
New Password:
The district user's personal FIMWeb password. The password may contain any alphanumeric values, for example, 0-10 or A-Z - and is NOT case sensitive.
Verify Password:
This entry must exactly match the password typed into the "New Password" field.
Save:
This button will save the contact information to the FIMWeb database and take the user to an abreviated adoption schedule reference for 2001-2002, from which they may proceed to the data entry screens.
Reset:
This button will reset all the fields to the original values that were present when the page loaded.
All fields on this form are required.
Update District User Information form for updating existing district users accounts.
The user must choose the "Update District User Information" link (available at the bottom of every page) to modify an existing district user account.

If the district user is logged in, this link will display the "Update District User Information" page.
If the user is not logged in, the link will direct the user to the "Login" page and require the user to log in to his/her existing account before modifying it. The "Forgot Password?" link provided on the "Login" page allows users who have forgetten their password to request it be sent to them via email.

First Name:
The first name of the person who will be entering data, as it appears in his/her user profile.
Last Name:
The last name of the person who will be entering data, as it appears in his/her user profile.
E-mail:
The email address of the person who will be entering data, as it appears in his/her user profile. The user must supply a valid e-mail address for DOE contact purposes.
Phone:
The area code and phone number of the person who will entering data, as it appears in his/her user profile.
The "New Password" field offers the user an opportunity to select a new alphanumeric password. TO RETAIN THE CURRENT PASSWORD, ENTER IT HERE. The password is NOT case sensitive.
This entry must exactly match the password typed into the "New Password" field.
All fields are required on this form.
Login form for logging into district user accounts.

District:
The district user must select their district from the "District" drop-down box.
Password:
The password chosen when setting up the district account.
Forgot Your Password:
Click this link to access to the "Forgot Your Password?" page. This page allows the user to receive his or her password via e-mail. This is the password that was entered in the "New District User Setup" form when first setting up the district account or on the "Update District User Information" form if the password has since been updated.
Forgot Your Password form for e-mailing forgotten passwords to district users.
If a district user forgets his or her password, FIMWeb will email the password directly to the user's e-mail address as it appears in their user profile.

District:
The district user must choose a district account that contains the password.
E-mail My Password To Me:
This button can be used to send an e-mail to the district user's e-mail address that was entered on the "New User Setup" or "Update District User Information" page. If you do not receive an e-mail from FIMWeb within 24 hours, please contact DOE at 1-800-307-6928.
